Maintenance errors cost operators of commercial aircraft millions of dollars each year in rework, on-time performance and lost revenue, and present potential safety concerns. For example, aviation industry studies indicate that over 20 percent of all in-flight engine shut downs and up to 50 percent of all engine-related flight delays and cancellations can be traced to maintenance error. In response, Boeing developed the Maintenance Error Decision Aid, MEDA process to assist maintenance organisations to identify why these events occur and how to prevent them in the future.

MEDA is a structured process for investigating the causes of errors made by maintenance technicians and inspectors. It provides a systematic and comprehensive process for conducting thorough and consistent investigations, determining causal factors and thereby enabling the organisation to learn from mistakes. Errors are a result of contributing factors in the workplace, most of which are under the control of management, therefore, improvements can be made to eliminate or mitigate these factors so that similar events do not reoccur.
